This form has been specifically designed for customers who can't make it to our store. Pease fill out as many of the following details so that we can help you find the perfect outfit for that special occasion, or to help you to create a perfectly co-ordinated wardrobe.

You can either email, fax or post this form to us. Please make sure that you fill in your name and your email so that we can reply.

If you have some fabric samples, you can mail them with this form and we will gladly match them with samples of our own or you may have a picture of an outfit that you wish to copy, so please feel free to post it as well.
