You can either email, fax, or post this form to us. Please make sure that you fill in your name and your email so that we can reply.

If you have some fabric samples you can mail them with this form and we will gladly match them with samples of our own or you may have a picture of an outfit that you wish to copy so please feel free to post it as well.
